2024 Annual Community Report (2023 Data)

A little about who we serve:

• We provided 25,575 visits to 5334 children and adults last year for Medical, Dental, and Behavioral Health needs. 3,265 of these were virtual.
• The patients we serve are comprised of 26% Medicare, 29% MaineCare, 29% private Insurance and 16% uninsured.
• We provided sliding fee discounts and other supports in excess of $92,935 supporting 27% of our patients who fall at or below the 2023 Federal Poverty guidelines

A little about how we measure up:

• EHC continues to maintain NCQA Certification for the Patient Centered Medical Home Model and reports regionally & nationally on many quality of care measures each quarter.
Here are a few of those quality measures:
- Diabetic Blood Sugar control: 86% have blood sugars in therapeutic range
- Statin therapy for Prevention of Cardiovascular Disease: 76% of our patients
- Cervical Cancer Screening: 71% of our patients have had Cervical Cancer Screening

A few things that we are proud to share from this past year:

• EHC is in growth mode. We continue to add new patients every day!
• We continue to build our care management services to support patients beyond the provider visit.
• Our Community Health Workers assisted nearly 500 patients with Sliding Fee Scale enrollment, accessing support services, health education and wellness promotion, and enrollment into insurance plans.
• EHC remained an essential community provider for Pandemic testing, vaccination, and care.
• We are now distributing free produce monthly in both Eastport and Machias to community members.
• We welcomed Robert Phares, MD; Fred Hartman, MD, and Sandra Manship, CFNP to our team.
• We opened our new EHC Machias building in July 2023 and are planning a new EHC Eastport facility!
