Community Health Workers (CHWs)
CHWs typically reside in the community they serve and are Facilitators. They meet community members where they reside and are frontline proxies of change, helping to reduce health disparities in rural communities.
CHWs support patients and community members alike with:
- Social Security and Social Security Disability Applications
- Mainecare applications
- SNAP/TANF assistance
- Job application processes and assistance gathering information needed for interview
- Food resources including food pantries, monthly Produce from Good Shepard, EBT applications, food bags and coordination with other community partners for additional resources
- Assistance with free/low-cost medical equipment
- Assistance with HEAP, winter fuel assistance and appointments
- CoverMe.gov certified
- Applications for transportation with Modivcare and other transport services, bus tickets, etc.
